Effect of Fibrin Glue on Epithelialization of Split Thickness Skin Graft Donor Sites

Though many surgeons have discussed the treatment of the split-thickness skin graft donor site, there is no standard method. Fibrin glue products consist of fibrinogen and thrombin, which play central role in blood coagulation and providing a provisional matrix for epidermal cell migration. The purpose of our study is to evaluate the effect of fibrin glue on epithelialization of split-thickness skin graft donor sites. Twenty patients with skin donor site wounds were enrolled in the study. The author designed a side-by-side matched-pair donor site wound model to compare fibrin glue with foam dressing and foam dressing only. The digital photographs were taken on 6th, 8th, 10th, 12th, 14th day after the procedure then evaluated and scored for the epithelialization. The day of the complete epithelialization and the visual analog scale (VAS) pain score were also measured. Donor sites treated with fibrin glue healed in 13.35¡¾5.75 days and those treated with only foam dressing healed in 13.55¡¾5.63 days (P=0.465). VAS score treated with fibrin glue was significantly lower than control group. Fibrin glue itself does not significantly affect epithelialization process. However, donor site treated with fibrin glue showed significantly lower VAS score, which means less pain for patients.
